BOOC's Case Management services include but not limited to assessment of ongoing support and service needs. We provide ongoing communication to Program Coordinator/Director and staff; Oversee the intake and discharge process, Initiate and maintain internal and external referrals; Maintain a filing system, progress notes and FAC data base.
BOOC provides ongoing communication to DCFS/POS case workers, lawyers and other service providers. We build relationships with specialized services that our clients need. We serve as liaison with DCFS and other discipline agencies provided to clients. BOOC's coordination of all services helps provide direct care to the clients and maintains client confidentiality at all times. This is done with ethical behavior per policies and procedures.
